Kim Alvarez has been with Angels Grace Hospice and Palliative Care for 5 years as CNA Marketing Liaison, working alongside a wonderful team including RN Case Managers, Chaplains, Social Workers, Volunteers, Pet Therapy, Reflexology, CNAs, Physicians, Bereavement Services, and Palliative Care.
Our RN Case Managers come up with a care plan for every patient, which includes their personal needs, medications, and what visits they will need. We work alongside the facility and their staff to ensure that the utmost respect and care is given.
