Scott Smith, a Registered Nurse (RN) at Angels Grace Hospice & Palliative Care of Illinois. He has been a hospice nurse for five years. Starting as a nurse case manager, Scott is now one of the hospice admission nurses.
Scott, along with our other hospice admission nurses, meets with and provides an evaluation of people who are considering their options for hospice care. This process is done in conjunction with the patient, their loved ones, their primary physician, and Angels Grace Hospice Medical Director. Once eligibility for hospice is established, Scott obtains consents for care of the patient and then the personalized plan of care is created, initiating the hospice services.
Scott visits hospitals, homes, assisted living facilities, and long-term care facilities as part of this process; each meeting usually takes 1-3 hours, providing ample time for an excellent explanation of the Angels Grace Hospice services, and time for patients and their loved ones to have all of their questions answered, and needs determined.
